Plan Information
Portability of this plan is only permitted under the following circumstances:
- Employee terminates employment
- Employee is no longer eligible due to class change
- The Cancer Policy ends
Individuals must be covered under the plan prior to the termination date in order to port the plan. Applications to port the plan to an individual policy must be submitted within 31 days of the date of the termed plan.
Should an employee pass away, the surviving spouse may port the coverage, or surviving spouse and children, as long as the spouse and children was/were covered on the date of the employee’s death. The spouse must be under the age of 70.
